How does a hospice program work with my personal physician?
Within a hospice program, it is important that you are surrounded by individuals who are familiar with your personal goals, your medical needs and your healthcare history. Therefore, your primary care physician is considered a key member of your care team and remains actively involved in your care. He or she works with your other hospice team members to review the course of your treatment and make recommendations about your care. Additionally, if a medical need arises that is unrelated to hospice care but is important to your quality of life, your physician will continue to manage your services.
The hospice physician, most times referred to as hospice medical director, oversees your hospice care. He or she helps establish a solid, effective relationship with you, your caregivers and your healthcare providers. The hospice medical director is responsible to ensure that the services you receive are in accordance with Medicare Hospice Benefit guidelines as well as the standards of care set by the hospice program you choose.
At Midwest Palliative & Hospice CareCenter, our hospice medical directors also are practicing consultants in the field of palliative medicine, the medical specialty that reflects advanced training and expertise in the care of patients with life-limiting or life-defining illnesses. We work alongside your doctor to provide whole-person care, address the complexities of care in advance illness and manage your pain and other symptoms, thus enhancing your quality of life. We work diligently to develop good communication between our organization and your doctor so that we can provide the best possible care. Continually improving teamwork helps ensure that the relationships with the people whom you find life-enhancing are protected and supported.
