Background & Accreditations

Focus of Services
  • Services are patient- and family-centered, focusing on the needs of both the patient and his/her family.
  • Palliative care is provided for individuals with a serious illness or at the end of life; support is provided to their families.
  • Palliative care is comprehensive specialized care provided by an interdisciplinary team that focuses on improving quality of life for patients and their families who are facing life-limiting or life-threatening illness. This is done by preventing, assessing and treating pain and other physical, psychosocial and spiritual concerns, whether the goal is cure or care.
  • Services that we provide include: hospice, palliative care consultations, grief support and special services for children.

Statistics
  • 4,500 individuals received comprehensive medical care in 2008, and hundreds of others received support services through counseling, crisis intervention and workshops.
  • The 2009 budget is nearly $30 million.
  • More than 300 staff and a cadre of 240+ volunteers fill a variety of roles—service delivery, leadership, administration and advocacy.

Where Services Are Provided
  • Approximately 50% of hospice patients receive services in their own home, though care can be provided wherever the person is residing.
  • Midwest CareCenter has contracts with more than 120 nursing homes and assisted-living facilities to provide home wellness services and hospice care to residents.
  • Other services can be provided at home, in our hospice inpatient unit, a retirement center, in the hospital or in a physician's office.

  • Skilled nursing facilities and other extended care settings.

History
  • Established in 1978 as Hospice of the North Shore.
  • Corporate office is located in Glenview, Illinois.
  • Regional offices are in Libertyville (Lake County) and Norwood Park (north/northwest Chicago).
  • Hospice inpatient unit, with 16 private beds, was established in 1995 and now is located on the campus of Skokie Hospital (formerly Rush North Shore Medical Center) in Skokie, Illinois.
  • Name was changed to Midwest Palliative & Hospice CareCenter in 2005 to reflect the expanded service area and the focus on palliative care for any stage of illness, not solely at the end of life.

Governance
  • Incorporated as a nonprofit organization in the state of Illinois.
  • Governed by a voluntary Board of Directors representing a variety of professions and connections in the community.
