To provide personal care and assistance with activities of daily living to patients under the supervision of the registered nurse consistent with Midwest CareCenter policies and the individual plan of care. Part-time (0.5 FTE) position in Skokie, IL.
Education
- High school graduate or equivalent.
- Graduate of a certified nursing assistant program.
- Maintain Illinois nursing assistant certification.
- CPR preferred.
Experience
- Demonstrated clinical ability through written and skill test.
Other Requirements
- Ability to work with patients and families in distress.
- Comfort working with the dying, patience, compassion.
- Good communication skills and ability to communicate with a wide variety of professionals.

Field Admission Nurse
To facilitate smooth access to services and admission to appropriate programs, by serving as a liaison between physicians, patients, families and facilities when needed. Two full-time positions in northern Cook and Lake counties.
Education
- ADN or BSN.
- Registered Nurse in Illinois.
- State of Illinois licensure.
Experience
- Minimum two years' experience in oncology/med surgical/home care/hospice.
- Ability to assess and apply critical thinking skills in areas such as physical assessment, disease physiology and disease progression, symptom management and pharmacology.
- Detailed knowledge of Midwest CareCenter programs and eligibility/administrative requirements.
- Knowledgeable of Medicare and Medicaid standards and eligibility for Midwest CareCenter programs.
- Computer literacy; specifically, MS Outlook, Allscripts.
- Knowledge of interpersonal and group dynamics.
- Knowledge of drugs and drug interactions.
Other Requirements
- Must have valid driver’s license.
- Must have unlimited access to car and maintain liability auto insurance.
- Ability to quickly establish rapport with patients, family and facility staff.
- Ability to transfer knowledge to skills required for patients to access appropriate care.
