Chaplain
Provide spiritual and emotional support for patients, families of patients, and caregivers. One full-time position in Chicago and the near north suburbs.
Education
- Master of divinity or related graduate level theological degree (MTS, MAPT, MAHL, etc.).
- Ordination/consecration or lay chaplain endorsed/licensed/canonically resident by denomination and of active status.
- Clinical Pastoral Education: at least one unit, four preferred.
Experience
- Previous experience as a chaplain in hospital/hospice setting preferred.
- Basic computer skills and willingness/capability to learn software applications and EMR documentation.
Other Requirements
- Must possess/use own vehicle, have valid driver’s license and maintain liability auto insurance.
- Strong communication and interpersonal skills.
- Ability to use a PDA, cell phone and computer.

Social Worker
To provide social work services to hospice patients, families and caregivers as well as to assist staff at related long-term-care facilities. Part-time (0.2 FTE), weekend position in Lake and northern Cook counties.
Education
- Master’s degree in social work.
- Licensed Clinical Social Work (LCSW) license.
Experience
- Minimum one year experience as a medical social worker.
- Familiarity with end-of-life processes, community resources, the Medicare Hospice Benefit; knowledge of family systems and family therapy.
- Knowledge of psychopathology, crisis intervention, conflict resolution and problem solving.
- Highly skilled in short-term therapy models.

- Ability to work independently in a complex environment; flexibility, patience and good listening skills.
- Excellent communication skills, diplomacy, compassion, empathy and collaborative capacity.

To provide anticipatory and bereavement services to both hospice and community families. To serve as a resource to the community for children’s grief and loss issues. Part time (0.6 FTE).
Education
- Master's degree in social work, counseling or related field.
- LCSW or LCPC license in Illinois.
Experience
- Clinical experience in individual and group counseling with children and families.
- Experience in healthcare setting preferred.
Other Requirements
- Strong interpersonal and communication skills.
- Knowledge of death, dying and grief and the impact on the family system.
- Understanding of the psychosocial dynamics operative in chronically and terminally ill patients and their families preferred.
- Knowledge of the developmental needs of grieving children preferred.
- Knowledge/willingness to work with various computer programs helpful.
- Unlimited access to a reliable car, current Illinois driver’s license and automobile liability insurance.
