South Suburbs Services Transferred to Horizon Hospice & Palliative Care
October 2008
Midwest Palliative & Hospice CareCenter transferred ownership of its south suburban office and employment of staff to Chicago-based Horizon Hospice & Palliative Care, effective October 1, 2008.

The office based in Olympia Fields will become the hub of south side operations for Horizon, which provides Medicare-certified hospice and palliative care services to patients and families facing serious illness and the end of life.

In addition to expanding its program, Horizon will continue to serve the community through grief support services for adults and children. It also will help connect seniors and youth in the community through the Kids Who Care program.

Like CareCenter, Horizon, a nonprofit community-based program, was founded by a group of committed volunteers who wanted to make a difference in the in the lives of patients and their families. They too have a deep commitment to help families and friend find support during their grief.
